Position Description
Spectrum Healthcare Resources has an Exciting Potential Opportunity for a RN Case Manager supporting The Veterans Administration Office of Community Care in Glendale, CO!
Qualifications:- U.S. Citizenship and residency in the U.S.
- Nursing degree from an accredited school (ACEN or CCNE).
- Active, unrestricted RN license in any U.S. state or territory.
- Minimum 2 years of case management experience in the last 3 years.
- Current BLS certification and strong computer skills.
- Experience with utilization review and care coordination.
- Maintain patient confidentiality and ensure a safe care environment following VA and Joint Commission guidelines.
- Review and process consult requests, ensuring completeness and appropriateness of care.
- Coordinate community care by reviewing medical records and communicating with providers, veterans, and staff.
- Attend team huddles, staff meetings, and complete clinical alerts in CPRS within seven days.
- Provide patient-centered care, assess needs, and develop/update care plans.
- Report changes in patient conditions and notify physicians as needed.
- Ensure compliance with HIPAA, PPE, hand hygiene, and VA policies.
- Respond to emergency situations, assist in admissions/discharges, and deliver lab specimens.
